At 08:00am i recivd00 AM, I received a phone call from Patrick Duke, and he informinformed me thethat he had had an accident,. I askasked Mr,. Duke if anyone got hurt and where are you,he was. He told me no"No" and that only the equipment got damaged. I went there and when i seeI saw the damaged i ameadly call sefetydamage I immediately called for safety and my MAnagermanager to come and look it thatat it. I ask toasked Mr,. Duke what happenhad happened and he explaingexplained to me that when he was parking the Front Loader, the right back door cem openopened and tuchtouched the tire and fall ofwhen it fell off to the ground.
