I would like to apply under section 80f of the Employment Rights Act 1996 to wfork a flexible working pattern different to my current working pattern.

This is because iI have parental responsibility fofr a child aged up to and including 16.

I am making this request so that I can take care of the child.

I would like to work Friday and Saturday nights only, and any day during school breaks.

I would like this woarkirangement to start from 07/10/2016.

To minimise the impact on colleagues and business, iI am willing to consider a relief/bank worker position.
